Rackmounted or desktop, brand not terribly important. I'm looking for a small mixer with monitor sends, three band minimum channel eq, mute if possible - and XLR outputs. Wouldn't have expected to have such a challenge.

Do you need XLR outs for everything (and if so, why?), or just the mains output?

Probably overkill for you channel-wise, but you could pick up something like the Mackie ONYX 1220 (can be had for under $300 used) and get a DB-25 to XLR output cable. The pres and EQ are pretty decent in those, though I think the EQ is bypassed if you use the DB-25 direct out. There is a mod available for that though so that the EQ is inline.

Might be some other options out there, but this is the first one I thought of and others might be more pricey.

Just XLR main out. Planning to use it for mixing two signals to my stage amp, and as an input for an MP3 player. XLR outs for sending a signal to the snake for FOH without having to buy another DI and have more cables cluttering things up.

I use an old Mackie 1202 VLZ for applications like that. It makes a great EFX mixer too. Should run you around $150 or less shipped if you are patient.

+1 on the mackies, or even behringers...some of their lower end stuff isnt half bad...for the price.
used tho the mackie 1202 is a good one...i think that the mackie preamps are decent, so its all good.

My jazz band uses a 1202 VLZ pro live. Great little mixer. XLR outs right to the srm 450's. Also, the rca ins and outs (for your MP3 &c.) work with consumer level (-10 dB) gear. I run the headphone out of a little discman during breaks and it sounds great.

I just got a Behringer UB1222FxPro for $150, it has 4 or 6 phantom powered XLR channels, 2 or 4 stereo channels, monitor out, efx bus, external audio bus, 7 band EQ, onboard effects, can be rackmounted, ships with ears and xlr main outs.
I'm using it in my studio so now everything is hooked up and ready to roll.

Another +1 on the Mackie 1202 VLZ. I've never grown out of mine and it's still in great shape after over 10 years of use.

grabbed a mackie 802 vlz3 and it's perfect for my needs. nice little mixer for the money, it's no midas or a&h but the price and features are right!
