Needs at least bass and treble (although I'd prefer a mid control as well) controls and an XLR output to run to a board. Must work well.

Have you considered a SansAmp Para driver? Bass/Treble/sweepable mid and drive controls. Good quality DI. In perfect shape. I might part with it for $200.

Try a Phil Jones Bass Buddy. For just over $300 you get a DI with so many more features. It is a great practice tool, and a nice sounding pre. I got mine from the Perfect Bass.
