Up for sale is a Bartolini 4.5 2 band EQ with push/pull volume for passive operation. Preamp is 1 vol (push/pull), 1 blend, and 1 bass tone, 1 treble tone which are defeated in passive operation.
I installed this on my MIJ Fender Jazz special without realizing the tone wouldn't work in passive operation - my previous G&L had spoiled me and I was clueless about that. So the shop that installed it replaced it with a pair of Tonestylers and a set of Noble volume and blend controls. So now I'm happy with my bass, and you can have this for a lot less than a new one. (Anywhere from 120-160 depending on the shop, judging by my Google search.)
Preamp was professionally installed, and is currently wired for 9-volt operation but I will include the second battery harness if you're interested in 18-volts of headroom. Includes washers for mounting and input jack - all you've got to do is drop it in and solder the pups into it.
