I just had a child so all excess gear has got to go.
It was made in Linden, NJ, by The Ampeg Company, and all the pots, original caps, and speaker date to 1970-1972. This amp has the factory upgraded Altec-Lansing 421A.
I purchased this amp about in late 2006 from the original owner, which was a church in SW Virginia. At that time the amp was completely restored: all the electrolytic capacitors were replaced, the out of specification resistors were replaced, the diode in the bias supply was replaced, one power tube socket was switched out, a 3-prong power cable was added, the polarity switch disabled (hence the 3-prong power cable), a 1/4" output jack was added, the speaker cable had a 1/4" end attached, all tubes were replaced (NOS JAN/Phillips preamp, JJ Power, Sovtek rectifier), the broken lid latches were replaced, and new 3-1/2" casters (2-lockable) were added to the dolly board. This amp is in perfect working order.
I am also including the vinyl dust cover that fliptops.net sells. It has no padding, but will help the tolex from being scuffed up.
