Mid Freq. 3 way select switch
Z mode switch
Active/Passive switch
9 volt battery with LED indicator.

Configurable......
This is not one of the Audere Jazz plates. It is was a configurable order with a Z-Mode switch - High Z, Mid Z and Low Z.
