Just pulled from my '97 LB75 and in perfect working condition. Controls are Volume (push-pull for Active/Passive), Blend, Bass, Treble, and Mid (non-sweepable). From reading on Carvin's site, this seems to be the 501B (there are no indicators on my preamp that states 501B).

This is for the preamp only: no pickups, output jack, battery, or any of the leads (everything in the carvin pic except for the output jack).
