This is a 5 string Jazz Pickup that is touted as "a custom designed noiseless", so I'm guessing it is not off the shelf. It came off my Fender Reggie Hamilton 5 string Custom Shop. $50

John Suhr Designed Preamp w/ control plate.
Active Mode:
Master Volume, Pickup Pan Pot, 3-Band Active EQ with:
Treble Boost/Cut: +/- 14 db @ 2kHz to 18 kHz with a peak at 8kHz,
Mid Boost/Cut: +/- 12 db @ 1kHz,
Bass Boost/Cut: +/- 15 db @ 30 kHz,
Passive Mode:
Master Volume, Pickup Pan Pot, Passive Tone Control
This preamp has been wired so that passive tone works in active and passive modes. $50
