It's the 3 separate pot version, but could be converted for the stacked bass/treb if you get the other type of pot (not included, I don't have one).
Push/pull Mid with resistors.
Includes washers, nuts.
Can also include battery clips if you need them.
You should use your present volume and/or pan controls, as they match with the pickup values, but I can check my gear box if you need anything else as well.
I also may have a jack, depending on what style your current one is.
