Pulled this out of a 5-string bass and no longer need it.

Consists of 4 pots - 2 regular and 2 stacked.
Mine is a little unusual in that it has independent volume controls (the "regular" pots). The stacked knobs I *believe* are treb/bass and high mid/low mid. Although I don't really remember since I haven't used it in a while - could be treb/high mid and bass/low mid or treb/low mid and bass/high mid. Any way, it's some combination of those frequency bands (pretty sure it's the first one as that would make the most sense.)
Comes with battery clip, jack, and knurled black metal knobs.... everything is pre-wired. The volume pots appear to have been swapped out for 500k non-original pots. Not sure why, the bass was used, and that's how it came. If you don't like the 500k pots, they'd be fairly easy to replace.
This preamp would run you $160+shipping new. Mine for
$105 shipped o.b.o. in the 48 lower U.S.A. Audere stuff is awesome. I have Audere Jazz-plate style pre-amps in two other basses.
