I got mine from pickguard paradise. I sent in the guard off my bass and he sent me this:

The way he does it is he prints whatever graphic onto a clear guard and then puts a white backing on it to protect the print and bring it out. Even on a black background it looks good.

You can also get a local luthier to make one for you if he/she has the tools. I went that route for a totally custom design, and it worked out great!
