HELP...Jazz bass wiring.

I recently put some new passive pickups in my Jazz...but now I can't get it working, I forgot how it was all wired up.

I'm looking for a guide or schematic for a simple volume/volume/tone pot setup.

Help, this is the only bass I've got & it's driving me nuts
