Please help with Buzz / Hum from wiring

My Jazz style bass has a severe hum when I turn the tone pot colckwise. Any suggestions on what the problem is, and what I can do to fix this.

I realize the Jazz basses will hum a little due to the single coils, but this happens even when both volume controls are set the same. It seems to be directly related to the tone pot.

I realize this has likely been asked countless times before, however the search function gives me almost 500 pages and my server isn't working too well right now.
