Problem with one of the toning knobs on my bass...

Basically one of the knobs on my Yamaha 5 stringer has come off. I know how to repair it but the issue that I have is that this is not the first time this has happened- this also happened with my first bass.
Does anyone know what the problem might be caused by? The bass this has happened to (my Yamaha 5 stringer) wasn't a cheap bass either...

A tone knob (dial) or a tuning knob (tuner)?

Is it split shaft? Might just need to bend the shaft out a little.
