Would it be at all practical to put block inlays in a fretboard that just has dots? Do you think that would be really expensive to have done? Is it even possible??
If it's real ones, it's expensive. The plastic ones come nowadays in liquid form that is really simple to use. You (luthier) just carve the desired shape and the liquid is poured in the cavity. When it sets, it takes a bit elbow grease to sand it to the required radius and to polish it back from the matt finish.
