Yes, it can be done. The amount of work it will take will depend somewhat on what condition the fingerboard is in. It's not something you should attempt yourself if you don't have the right tools and some experience. In many cases it's cheaper to buy a replacement neck, assuming your bass accepts one and they are available.
I'd agree with the replacement neck thing unless the instrument is really worth keeping original. Our guitarist had the bottom 5 frets replaced on a guitar and paid something close to £10 a fret! He figured it wasn't unreasonable for his favourite guitar which he'd had for 8 years.

Yes, that's about the going rate for fret replacement around here by a reputable shop. There are a few guys out there working really cheap who do a butcher job so you're better off checking around to find who the working pros go to.
