I usually stick a pair of needle nose pliers in a sock so I don't scratch up the peg. Grip it around the neck of the peg and pull up while holding the guitar body down. If it's in tighter, I just carefully stick my hand in the sound hole and push up on the peg with my finger or the face of a quarter from the inside. I haven't damaged anything yet.

Needle nose and something for leverage. I have also used a church key before.

Hi.

On tight/large ones I use wire cutters with tape- or plastic covered jaws.

Regards
Sam
