VINTAGE JAZZ NECK: ID and repair advice needed.

Hopefully you guys can help me out: I have a 60s or 70s jazz neck that has way too much relief in the neck (the neck really kicks up at about the 14th fret). Firstly, can anyone help me get a year for it? (there's no serial or stamp anywhere on it including the heel). It's a 4 bolt. The truss rod style is ambiguous as it's had a poor repair done before.

I'm guessing it's had a broken truss rod at some stage. As you can see in the photos there is a veneer over the first fret region of the fingerboard (see how the binding is cut away at the surface of the first fret?). The truss rod nut/dowel cavity area has been filled with some sort of glue that's now fairly soft. At the heel end of the neck there's a crudely drilled hole for truss rod nut access. I'm wondering if someones switched it from an originally headstock end adjustment to a heel end adjustment.

I'm reasonably experienced at repair, but I wanted some input:

Is the neck worth repairing and how should I go about it?
