I was doing some "window shopping" over at MF just now and I saw an item that got the old mental gears turning. It's a Rolls PM55 Personal Monitor Amp +, and as I was reading the description it got me to thinking, what if you could run the signal that ordinarily goes to a stage monitor and run it through this box instead, and plug in some earbuds in the headphone plugs. I own a similar headphone amp which I use for IEM, though mine is a rackmount version (in case some of my bandmates also want to use it).
There is one issue though: with a wired IEM, you then have two cords running to/from you. This does present a possibility of entanglement.
However, I found a good device I outlined in this thread.
It's a beltpack that regulates the signal from the headphone amp, and has a single proprietary cable that breaks out into a headphone TRS jack (for the signal from the headphone amp) and an instrument jack (for your bass signal into your bass amp).
I've used it on a number of gigs. The cable is a little stiff, but it otherwise performs flawlessly. I get to mix my own IEM signal from the monitor feed and my bass DI so that I have my vocals and some bass in the IEMs.

If you play an instrument, this device Rolls PM351 would give you the added 'more me' volume control of the instrument as well as the mic/vocal. The rolls device you linked has a control for mic + monitor. This one has control for mic + instrument + monitor.

Essentially, every musician/vocalist has a personal mixer with volume and L/R balance for up to 15 instruments/microphones, into which we plug headphones. So, I've got bass, electric guitar 1 and 2, keys 1 and 2, acoustic guitar 1 and 2, drums, kick, vocals 1-4, spare mic, and soundman talkback. I imagine this thing you're looking at is similar. You can get a wireless pack and run off of your Aviom mixer. If for church purposes, it's something to look into. For regular gigging, something more portable (like what you've got here) would work well.

I'm sure they're great systems, and I'd love to play with one, but we're going to have to sell a ton of CDs before our band can afford one!

I decided that having two cords coming from me was an invitation for disaster. I wanted to go wireless, but was unwilling to spend the $$$, as I was worried about interference. Another huge issue for me with wireless is that on the small stages we often play, I sometimes end up standing right next to my rig, and some wireless devices have major problems if you get the receiver too close to the transmitter. Most want at least 5-10 feet minimum distance, which isn't workable for me on some stages.
Thus the JumpAudio box I wrote about in the other thread was the right solution for me, and it wasn't too expensive, either. It allows me to have the reliability of a wired system but yet still only have a single cable to deal with on stage so I'm not getting tangled up in my bass and IEM cords. And no batteries to deal with either. YMMV.

I do us IEM though most times. i use an inexpensive mixer but the rolls would be fine. I took the two cables and taped them togheter at 2 foot intervals. That still leaves flexibility but in rality I'm only watching out for one cable. It is cheaper as well.
