My middle finger is seldom used

Is this strange?
I usually play scales in... how do I put this....
D-1-3
A-1-3
E-1-4

Make sense? I hardly use a 2 unless I change the same scale into:
D-3-5
A-1-3-5
E-3-5

Then I used my middle finger to play the 3s.
Is this abnormal? Inefficient?

Playing with all your fingers is potentially the fastest =p But whatever works for you will do just fine.

So what if you want to play the other 2 notes?

According to my makeshift scale, where would the 2 phantom notes be?

I use my middle finger all the time. As a matter of fact, I'm using it right now.

Mine is my main anchor point for anything Major. Index finger for minor. Pinky is secondary for both. I hope that makes sense. I have gotten a little too used to it though, because there are times when I should start from the pinky to optimize "finger per fret." For instance when playing I,m3,M3,5,etc. together in an arpeggio, I should start with the pinky on the 1 & play it 4 per fret, but I seem to have developed the lazy habit of starting from the middle finger & incorporating a 1/2 step shift for the threes(min & Maj), & it doesn't always fit the song.

You should be able to use all 4 fingers.
