Playing with a pen in the right hand: not easy, but it doesn't make playing utterly impossible.

Playing with a pen in the left hand: again, awkward, but not impossible.

Using a pen to play with the right hand: a pen doesn't make a good plectrum...

Using a pen to play with the left hand: The body isn't hard enough to use as a slide, and is completely impractical and awkward to use as a fretting tool.

Hmmmm, I don't have a problem holding a pen in either hand while playing. I'm left-handed (though I play a righty bass), so I often hold a pen in that hand while writing music. Actually it doesn't inhibit any movement whatsoever.

I've played using a drum-stick (kind of similar) it works pretty well

I once saw a guy play saxophone with a cigarette between his index and middle fingers...He did quite well, I guess you could say he was smokin'.
