I have just had this on one bass for about 6 months. It works perfectly. Comes with box and all papers. The jack has velcro on it so that it can be secured to the tailpiece with a cable tie. Of course, you can still use the string clip method too. One of the foam things is homemade and it has a hole rather than a slice so the wire would stay in place better.
I put shrink tubing on the rca plug to guide the cable away from the bass body-it would touch on my bass otherwise. It also protects the cable from strain too. I also am including an adapter kit (see inset photo) so that it can be installed on a bass with 3/8 adjusters!
