This is a very gently used (never left apartment) Ernie Ball volume pedal. Built to withstand a nuclear blast (...almost!), this is the slightly more compact, Junior, version of their famed 25K volume pedal. This can also function as an expression/parameter control for many other pedals, like the Eventide 'factor pedals, which is what I used this for exclusively. I do not have the original box for this pedal, though I remember it being plain and nothing to really miss.
