I am no longer using this unit but it always worked great for me when I did. It includes the original box, manual, receiver, transmitter and cable along with a few custom extras.
I made two strap pouches that can be attached to your strap (which I prefer over using the belt clip) and I also purchased two extra high quality transmitter cables in different lengths with right angle plugs. I am also throwing in a Boss AC adapter that works great with the receiver module.
