Bass scale sheet w/music notation for 5 string

I can read treble clef (former trumpet player). I would like to be able to read bass cleff quickly, and I want to learn the notes in each scale. Is there anywhere on the web that has a printable scale sheet with standard music notations (can be labeled with note names) that is designed for a 5 string?

I can't fine any!

As far as the notes on the bass clef, there are some acronyms for learning the clef (please note: this is not a tab sheet but the bass clef)

Here is another thread that has a nice cheat sheet. As far as the fingerboard goes, I don't have a nice example at hand for 5 string. The scales can be moved anywhere on the fingerboard - it's the same pattern no matter where you play it. Look at this guy's cheat sheet for the scale patterns. Then, search around the TalkBass site to find more information.

you probably wont find sheet music designed for a five string, just standard bass clef with leger lines leading down to the lower notes that a 5 string provides.
