I've been using Bryan R. Tyler's. He is Mod here on talkbass and has a very simple one that can be used on a four or five string +.

There is another great flash site somewhere that has harmonic and fretted chords. You can click a chord, it will show you the fingering position and play the chord... but I can't seem to find it, maybe someone will know what I'm talking about and post a link if I can't find it =)

Why would you need to know how to play chords on bass? You'll have to excuse my ignorance but i've done just fine for a really long time without every having to play a chord. I thought playing chords was for guitar/keys...

Not always applicable in every style. There's "doing just fine" in one style and there's striving for proficiency in every style and learning every technique relative to your instrument. The goal of the latter is mastering your instrument to the best of your god-given capabilities. The goal of the former - getting by - well, it is up to each individual what they strive towards.

99% of songs out there (I hate to go absolute, but this is pretty damn close) follow some sort of chord structure. Even if it's a good ol' fashioned I-IV-V, it's still helpful to know what notes are in each chord, or even just what flavor it is (major, minor, dim etc..) Helps greatly if you want to go beyond a root and 5th

Lotus, not sure if this is what you're looking for but I use this often for drop-down and unusual tunings...lots of options for printing out charts, etc...

I also posted a couple of lessons on this topic on the lessons section of my site.

Somebody above questioned the need for learning to play chords on bass, and I partially agree. At the beginner level, it's certainly not a high priority, and you will almost never need to do it on a gig, but for practicing, transcribing, and learning the sound of different chords, it's very useful IMO.
