Decent Instruction for Teaching Onesself to Read Music

Playing for a while but I want to know what are some ways I can teach myself/learn how to read music.

Books?
Websites?
DVD's?

I want to learn enough so that I might be able to sight read new material and perform something on the spot

Whats the best and most efficient way to go about this solo?

Must add, for sight-reading, I learnt this by doing it. At worship band I'd be given a song and have only a few minutes to read it before playing it.

Best thing to do for this is to build it up gradually. Take a song you know is at your ability level and then take a few bars out of the song, and try and play these straight off the bat. Check this against a recording, get someone else to play it etc. Then do the same with the other pieces of the song, building up the length and also speed if necessary until you can play the song.You could try this with a song you've heard but not played.

Reading music

Reading music isn't difficult... but you have to do it A LOT! Just learn how to read the notes on the staff and then get as much material as you can. Anything will do really as long as you read AND play it daily. I like Rufus Reid's "Evolving Bassist" and Jim Stinnett's "Creating Jazz Bass Lines."

I guess my point is that you must "practice" reading often. Just like when you learned written English. Read Read Read. It may seem boring at first but you'll notice a huge difference in your playing and your understanding of music.
