Check out some of the materials on the Jamie Abersold Website. Sight-reading practice for me was done in certain classes at Music School. I was in a Big-Band class in junior college and when we were deciding on what to play in our upcoming concerts, we did many hours of sight-reading. Maybe study with a pianist or a guitar player. Any ensamble playing involving reading and performing on the spot can help.

The ONLY way, and I repeat ONLY way to get better at sight reading is to practice sight reading. This is assuming that you are already skillful at normal reading. Get your hands on as much music as you can, and play it. Get a "rhythm vocabulary" or simply a drum instruction book and practice sight reading that if you find rhythms difficult to sight read.
