Does anyone notice a difference in the way the E string plays or sounds with a closed off extension vs. no extension at all?

Well, for Pizz slamming an open E, the regular Nut is usually better for raw type growl. The Ext/capo E is more like a finger holding it down, and is usually more earthy sounding.

Definitely so. I've even made the capo fly open from hitting the "open" E so hard--and of course I was left with whatever note was stopped, the low Db if I recall. Accidental re-harmonization!

I've actually found no effective difference if the E gate was well made and adjusted. I'm not just talking about my own stuff. There must be some damping effect, but I haven't really noticed it on my instruments. This is one of the reasons I argue with the "high nut" crowd. If you need nut clearance, then how is it that I am able to hammer on my E with the gate closed?

That is a big issue for me. I like to beat the low E up when I get the chance. I can't have a gate swinging open or the sound of a dull thud on that open string.

Suppose it's just a matter of how it's done?

Yeah, I think it's just a question of fitting the gate really well. The right angles, having good rigid mounting hardware, etc. I can open up my E gate by plucking if I really sit there and work at it, but in performance it's only happened when I didn't close it properly in the first place. Some people might get some rattle on the FB, but I don't have that problem either. And I do blast away on a gated note from time to time.
