Custom Rackmout Patch Bay- Who Makes Them?

Curious if anyone knows who might make these on the cheap. I basically just want a single rackspace unit with some slots for 1/4", XLR, Speakon, an Ethernet port and a 7-pin MIDI jack so I plug and unplug stuff much more easily than reaching into my rack . If I could solder or do wiring, I'd just get one of those knockout panels that several companies make and do it myself (like this one):

I know L.A. Sound Design. does them, but from what I've read they're $300+, which is a bit much for me.

I think Middle Atlantic might also do that sort of thing, not positive. http://www.middleatlantic.com/
