How to mount in a rack?

Trivial question I know but: when racking an amp head etc into a rack, in what order do I assemble these components: Screw, washer, head, rail?

You put the unit in the rack, make sure the gaps in the unit are lined up with the rail bits if its the sliding type, put the screw through a washer then screw it in.

You can add another washer between the rail and the unit, but, ive never felt the need to
