Songwriter does most things you need for about $40 (you get $10 off for upgrading from the free version of notepad). Of all things the thing I found missing was being able to add double bars - trivial but anoying.
Finale PrintMusic ($90 I think) does everything I want - double bars, plus you can add annotations like dynamics, and tempo changes. It's a bit quirky at times, but it's getting the job done. I think both songwriter and printmusic have real time midi entry.
You can try them all out for free. I've been using the free version of printmusic for the last few weeks to write out the bass parts for a show I'm doing. At the end of the 30 day trial I'll probably pay up. The Finale upgrade path is excellent - you basically can get to the next version for the difference in price, and the low end versions are pretty well priced.
