Edit as of 12-16-2008 Mozilla Firefox users, please refer to the following information.
A script has been created for the Greasemonkey application which will automatically convert any YouTube video to the highest quality sourcecode available. Once you install greasemonkey, it should detect the script and install it. If you have a problem with the script, leave a comment on the script page describing the exact problem, and they should fix the code accordingly. Be sure to check back to that page for updates in the script every so often.
-------------------------------------------------------------------------- For users of other browsers, refer to the following information.
If you have a YouTube account, go to My Account, and under Playback Setup, select option #3.
Many videos, as you may have noticed, do not have the link at the bottom-right to select high quality. For these videos, take a look at the URL. If the link contains an "&", erase anything after it such as "feature=related", or "NR=1" and add fmt=18 For 95% of videos on YouTube, this code will improve the pixel quality and sound quality.
Example: http://www.youtube.com/watch?v=uSxUFyNOwDg&fmt=18
Unfortunately, there are some videos on YouTube which for some reason are not able to receive an enhancement code. A few other more recent videos have also been uploaded from a 1080p or 720p source, and require a different code. That code is &fmt=22.
Example: http://www.youtube.com/watch?v=Qs20MGhs3S8&fmt=22
There is usually no way to tell which videos will take which code, so experiment!
Hope this helps,
-Sean

You're getting closer to the original experience by adding the code(s).
Anyway, I put the info out, so you can choose to use it or not to use it. But I would suggest you try it before you decide it's not worth the trouble.

Also, some (but not many) videos have been uploaded in 1080p or 720p, and require a different code. I believe that code is &fmt=22, and that will give you true HD if what I have read is true. | How does one go about trying this code? Got a step by step guide? |

First of all, what is the difference between standard quality and HQ? Well, it should be obvious when you click the link, but in comparison, HQ mode features sharper pixels, improved sound quality and stereo availability.
In order to watch videos in HQ mode without having to click on the link at the bottom right of the video every time, go to My Account (if you have one) and find Video Playback Quality under Account and select option #3. It says that you should choose based on your internet speed, but I find that some videos actually load faster in HQ mode than in standard. I'm not convinced internet speed makes much difference, if any.
Not every video has the option to watch in high quality mode, as you may have noticed. Or so you thought.
YouTube has created an awesome code which can be used for those videos which do not apparently offer the option.
The first thing you want to do is to check if the URL has a "&feature=related" or something similar, for instance: http://www.youtube.com/watch?v=aUQzXQhAgvw&NR=1
If it does, erase anything after the "&". If it doesn't, add the & to the link. Then, type in the phrase fmt=18
Difference: http://www.youtube.com/watch?v=aUQzXQhAgvw&fmt=18
You now are able to view this or any video on YouTube in HQ stereo mode! Sometimes the difference can be amazing.
Hope this helps,
-Sean | Thanks a lot! I'd heard about this before but never looked into it! Now that you've posted such clear instructions, I'm actually going to go about doing it! Quote:

I mean if you think about it, YouTube is a huge convenience like nothing before it, but really now, it's been QUITE slow as of late, at least I've noticed.
EDIT: So I did this, and it's pretty cool, but I found a video where the option isn't given, I don't believe, and it seems there's no difference when I add the "&fmt=18" http://www.youtube.com/watch?v=h3xICZZs2pk
