I've had the tip of a 1/4 in. plug stay behind inside an amp. Fortunately, I noticed it and removed the stray metal part from my amp's input jack before it rolled around causing mayhem!

If it had to happen, I'd rather it happen to an instrument, rather than an amp!!!

Happened to my amp. I dipped the threaded part of a bicycle spoke into Gorilla glue (just a tiny bit!), and inserted it into the hole on the little spike head piece left behind. Wait two hours, yank it out. No surgery!
