It usually happens when there is a mistake in the correction, but if something else is discovered, there can be disciplinary measures taken against the teacher. Maybe she keeps doing it because no one complained yet.

Email her, point out exactly the coincidence you mentioned to us, and politely ask for approval of your topic, pointing out that she has not yet responded to it. Mention that you have a deadline and have no option but to proceed under the assumption that she has approved your topic, since she has not commented on it.
Your communication needs to be time-stamped before the due date and time, so do it NOW.
And after this, interact early and often. Those are the ground rules, so you have to go with them. In the graduate class I teach, interaction is 25% of the grade, so I absolutely require interaction from each and every student throughout the course.
This may not be your strong suit, but learning to interact and provide feedback is a vital skill in the workplace and daily life. You need to demonstrate that you are on board. This may or may not solve the problem, but if you get a grade below that which you you earned, you need to have everything documented and be ready to go to the department head with your case.
