You can try it without, you'll just have to make sure you have a good seal around it and that you don't let it close when the "secret gases" hit it - unless you like indoor rainfall.

Back to back sinks are awesome, too, but that's more of a commercial thing. I've heard stories of guys using an air ram and then finding out a room next door got hammered with a blast of water and sludge.
I prefer the air ram because you can just fill it with air and vary your psi to use the minimum instead of hammering the system. Hard to beat a sink/drain cleaning machine if you can only have one tool, but these drain blasters can save some time and work when they get the job done. I'd much rather see homeowners use this than chemicals.
