Where are the serial and model numbers located for the Opto Stomp? I'm filling out the 5 year warranty card and it asks for both a serial and model number. There's nothing on the back of the pedal. Is it written somewhere on the inside of the pedal, or did they forget to apply the stickers to the back of mine? (Bought new from GC.com)

yeah, mine were on the box, on a white sticker with a bar code
