I have a sansamp on my board at the end, and a 20 ft cable going to my amp.

Do I need a buffer or is the sansamp exactly what I need?

Are you having any problems with treble loss using this setup? If not you don't need anything.

The Sansamp is buffered, but I'm pretty sure the bypass is not, so when it's off you don't have a buffer at the end of your chain.

No, you shouldn't need an additional buffer. The signal is fully buffered when active and when bypassed. The SansAmp buffer is actually pretty decent, imo.
