I had one and I'm pretty sure it can't be run at 18v's

Polish Love was a red llama, right? If it is, it uses a CD4049UBE and the datasheet for that chip says it's safe up to 20V supply voltage. Give it a try.

I still have one of those...might try it at 18.
What effect might one expect though and what are the potential problems that may show up?

I strongly suggest that you contact the manufacturer of the pedal before doubling the supply voltage. There are other components that can be severely damaged by a higher voltage, like LED’s and capacitors for instance.

+1 I think I inquired about running mine at 18v and I'm pretty sure the answer was no the chip could very well survive but I'm not sure about the surrounding components?
