What does speaker sim actually do?

Is it just a fancy name for an eq? Or does it involve a more sophisticated process?

it's "supposed" to emulate the sounds of various brands of speakers.
Basically yeah, a fancy name for EQ's

It was fun listening to all the samples. Various amp sims sound like different eq settings to me.

I believe there are both "mic phasing" and "multiple speaker driver phasing" effects at play in the better sims, not just simple EQ tricks going on.

These are critical into tricking the ear that one is listening to a real cabinet IMHO.

There is supposed to be a very aural difference between them. Different brands will do this different ways. Newer line 6 products use samples instead of EQ.
