I have this preamp and I can't remember what I pulled it from, and I remember when I did pull it I had no idea what preamp it was anyways. All I know is that the pots are nice and have a push/pull option for bypass built in.

demeter preamp maybe? I know they use a blue sleeve on the board not clear, but hey this is a free guess

I tried to read it once but the plastic is not transparent enough... Hoping someone would recognize it. I guess not!

Take the plastic off, you can always replace it with shrink tube or something.
