What kind of bass is Jaco playing?

I really want to know what bass this is, mainly because of the interesting headstock that looks like a double bass headstock (1:12 in the video).

Really amazing to hear Jaco actually "fudge" something. Granted, what he is doing is difficult as hell....

That's his famous instructional video, where he was dragged out of a mental hospital and cleaned up over a few days - very sad!

He was doing this with Jerry Jemmott and that's his bass that Jaco is playing there - it was custom made for him by Abe Rivera.
