How do I fix this problem?

The volumes don't work independent of each other. If you turn one down it turns the total volume down, not just for that pick up.

So all of the black lines are connected?

That should have been a little clearer.

Connecting ground between the two pots, ground from each pot going to jack, and one ground going to bridge.

    Is that clear?

My thinking is a bit muddled ATM having just gotten out of class, but perhaps the grounds should not be connected to the third leg of each pot. Try that.

Also I believe the commons from the pups should be connected directly together, and not to the ground common of the pots/jack.

  5. Switch the wires around so that the wire to the jack is on the outer solder terminals of the pots and the wires from the pickups are on the centre terminal.

As it stands, turning one pot all the way down will result in the signal at the output jack being shorted to ground.
