  5. That's a custom. I forget who it's by, but that bass (or something VERY similar) is on ebay right now and it says who the luthier is on the auction page. Just search "custom bass" and you'll find it.

    although they usually have an extended headless neck end.

    The luthier is Bob Frenz (Columbus, Ohio).

    says so right in the ebay auction.
    He makes some funky multi-laminate instruments.
