Does anyone know what the controls were on these? They had three knobs (John Entwistle's had a switch).

According to Barry Matthews, who has just written a book on the subject using impeccable sources:

"The one with Three knobs on was the bass Pictured with John Entwistle on the mag. Beat instrumental in 1967 I think. Johns Guitar Tech came up with the bright Idea to put an Extra Jack socket and vol control on the bass Have half the the precision pickup going through one amp & the other half going through another, and do away with the split Lead that John used, sadly John said yes and it sounded awful, the theory was you could have Volume control over two amps from the bass, it didnt work John asked him to put it back as it was, He couldnt. so thats the account of the Three knobs on a 66 slab.
