Identify This Bass

I just saw an article by Victor Broden in Premier Guitar. He talks about this bass as "a 5-string with a Jazz-bass body—nothing out of the ordinary... a Music Man-style splittable humbucker in the traditional Jazz bridge position (instead of the slightly more forward StingRay position, where you often find this pickup) and a Jazz bass pickup in the neck position."
The pickups and/or the bridge could have been switched out. Can anyone identify with certainty what it is?
