Paging David Raven Moon re: EB2 6 position switch

Hi David,
I picked up an EB2 with a Model 1 and no pots or switch. I read about your setup and wonder if you'd share how you wired yours. A diagram would be great if you could, but a description will suffice. I'm mostly curious about what value capacitors you used, whether you used the choke(and how if you did), and any changes you'd make if you were doing it today.
