Yep that did it! I used the wiring diagram from another thread on the forum (http://www.talkbass.com/forum/showthread.php?t=505054), and sure enough it was exactly what I needed. I now have a properly working bass again! Thanks again guys!

Awesome...yeah looks like this is indeed an Epiphone Rock Bass, and the wiring diagram for the jazz bass might just do the trick. I'm going to pull the soldering gun out tonight and will give it a try. Thanks so much guys!
