I'm pretty sure its inside the amp. Jack, wire and piezos are ok. I made this design on strip-board (without the vol pot): And I had to split it in halfs to make it fit the box. So I have to check everything again.

My main amp is a Little Giant from Ashdown. The low input has an impedance of 1M, the high input is 10k. I also have a GK1001RB-II collecting dust :) Right now I have to troubleshoot the preamp as no sound is coming through when I tap the piezos. Only hum.

Both layers are connected, the solder is hidden behind the wire. In the layout for ver. 2 Fdeck has put a 10k pot in line with the output. Pin 3 to circuit out, pin 2 to jack and pin 1 to ground.
