Regarding replacing that bridge: The Hipshot website has very detailed diagrams with all the measurements. Any questions on hole placement should get answered with those diagrams.

That's an old Schaller design. The BB-3535-003. Pretty sure it's discontinued. The few spots it is listed show them as "sold out" except one site. Not sure that site is legit. At any rate it would require buying the entire bridge. BB-3535-003 5-String Bass Bridge, Black At least you have a model...
