I've tried 4 different preamps in my bass, and this has been my favorite - but I find that I prefer the sound of my pickups passive.
So I removed this preamp from my bass, but I left all of the EQ pots attached - which would make for a pretty simple hookup:

I installed the capacitors for a 400Hz mid frequency, but you could remove them to have a 1k mid frequency or replace them to set it at your preferred frequency.

- Connect the battery to red and black
- Connect the white wire to the output
- Connect the output from your pickups, blend knob, or volume pot to the blue wire
